Process for Becoming a Foster Heart Link Mentor

In accordance with the Foster Heart Link Mentor Program screening policy, all Mentor prospects must complete the steps below to determine if a candidate qualifies to become a mentor.

  1. The applicant must return all completed materials in the application packet:
    • Application | Download Application
    • Copy of Driver’s License
    • Proof of Auto Insurance
    • Personal References request form
    • Pastor Referral request form
    • Interest Survey
  2. Schedule Training
    • There is an initial 4-hour training required. Training is scheduled periodically throughout the year in various locations.
    • Alternative training may be discussed.
  3. Arrange and have fingerprinting done with required forms signed.
    • Information Release
    • Criminal History Release Form
  4. Arrange a personal interview with Mentor staff upon completing all necessary forms and criminal background screening.
