Become a Mentor
Process for Becoming a Foster Heart Link Mentor
In accordance with the Foster Heart Link Mentor Program screening policy, all Mentor prospects must complete the steps below to determine if a candidate qualifies to become a mentor.
- The applicant must return all completed materials in the application packet:
- Application | Download Application
- Copy of Driver’s License
- Proof of Auto Insurance
- Personal References request form
- Pastor Referral request form
- Interest Survey
- Schedule Training
- There is an initial 4-hour training required. Training is scheduled periodically throughout the year in various locations.
- Alternative training may be discussed.
- Arrange and have fingerprinting done with required forms signed.
- Information Release
- Criminal History Release Form
- Arrange a personal interview with Mentor staff upon completing all necessary forms and criminal background screening.
